Support Coordination

What is Support Coordination?

Support Coordination can be included in your NDIS Plan to assist you with making the best use of your funding and implement supports suited to your individual needs.

Support Coordination can help you:

  • Understand and use your NDIS Plan to pursue your goals
  • Connect you with NDIS Providers, community, mainstream and other government services
  • Build your confidence and skills to use and coordinate your supports

Our Point of Difference

·         We deliver face to face services in your community and have an office near you!

·         Language barriers do not exist – we have local staff that speak fluent local language and dialects

·         We source culturally appropriate NDIS services for our participants

·         We support and understand connection to community and country

·         We provide advocacy support to participants and their families

·         We value the importance of building NDIS capacity and knowledge with our participants and their families

·         Our staff have extensive knowledge of community services, organisations, and resources accessible to you

·         Xtremecare staff understand geographical logistics and freight service delivery and we can assist you in finding the right equipment for where you live!

·         We promote participant engagement by connecting participants and service providers on the ground in communities

·        We have the ability work collaboratively with stakeholders such as Department of Health, Department of Education, Department of Housing and other community based services to ensure a holistic approach in delivering services to you

Our commitment

At Xtremecare, we are committed to building a local workforce and employing local community members as we have acknowledged that language and culture are at the centre of NDIS service delivery in our rural and remote communities. We ensure our Support Coordinators are trained and supported to deliver a quality service, whilst also considering your geographical location and cultural requirements. Xtremecare staff members operate with community needs and meaningful outcomes at the centre of our approach, regularly demonstrating a strong understanding of cultural safety and community protocols in service delivery.

We have experienced staff that can assist with all your enquiries. Please call 07 5514 6500 or email admin@xtremecare.com.au. We would like to hear from you, your families and your community.
